Bloom Holistic Health offers Direct Primary Care memberships.

Membership is designed to give you ongoing, relationship-centered primary care with more time, connection, and support.

How Direct Primary Care Works

Direct Primary Care is a simple, membership-based approach to healthcare. Instead of billing insurance for each visit, you pay a monthly membership fee for ongoing primary care with your provider.

Bloom membership is not health insurance and does not replace coverage for emergencies, hospital care, specialists, imaging, or services outside the practice. Many patients choose to use Direct Primary Care alongside insurance or other coverage for care that falls outside of Bloom.

Your membership includes:

  • Annual wellness visit
    A yearly preventive visit to review your health, risks, screening needs, and wellness goals.

  • Annual preventive lab panel
    One included yearly lab panel, when appropriate, using routine screening labs selected based on your age, health history, and care needs.

  • Primary care visits
    Ongoing visits for chronic conditions, medication management, health concerns, prevention, and everyday primary care needs.

  • Sick visits and minor urgent concerns
    Care for common illnesses and minor injuries, such as sore throat, cough, urinary symptoms, rashes, minor sprains, and other non-emergency concerns.

  • In-person and telehealth visits
    Visit options may include in-person care or telehealth, depending on what is appropriate for your concern.

  • Women’s health primary care
    Support for women’s health concerns, preventive counseling, and common women’s health needs.

  • Medication review and management
    Review of current medications, refills when appropriate, and medication adjustments as part of your care plan.

  • Care coordination and referrals
    Help reviewing outside records, coordinating care, and placing referrals when specialty care or outside testing is needed.

  • Direct access for non-emergency care
    Easier communication with your provider for non-emergency questions and concerns, within practice communication guidelines.

Membership Details:

First Member of Household

$75/month

Each Additional Member of Household

$35/month

Pay Annually and Save 10%

First member:

$810 annually

Each Additional Member:

$378 annually

How to Sign Up

  1. Click on the Book Appointment button

  2. Choose a “Member” appointment and complete all the fields

    • For your first visit choose an appointment type that includes “new member”

    • For all subsequent visits choose any member appointment
